|
비타민 C는 아스코르브산(ascorbic acid)이라고도 하며, 인체의 정상 기능을 위해 필수적인 수용성 비타민이다. 대부분의 포유류는 필요한 만큼의 비타민 C를 스스로 생성하지만 사람은 이와는 달리 비타민 C 생성 능력이 없으므로 식품에서 섭취해야 한다.
기능 Function
비타민 C는 혈관, 힘줄(tendon), 인대(ligament), 뼈의 중요한 구성 성분인 아교질(collagen, 콜라겐) 합성에 필요하다. 비타민 C는 또한 뇌 기능에 필수적이며 기분(mood)에 영향을 미치는 신경전달물질인 노르에피네프린(norepinephrine)의 합성에 중요한 역할을 담당한다. 또한 비타민 C는 카르니틴(carnitine) 합성에 필요한데, 이 물질은 지방(fat)을 세포 소기관인 미토콘드리아(mitochondria)로 이동(transport)시켜 에너지로 전환시키는 데 필수적이다.
비타민 C는 매우 효과적인 항산화제(antioxidant)이다. 소량의 비타민 C 조차도 체내의 없어서는 안되는 분자들인 단백질, 지질(지방), 탄수화물, 그리고 핵산(DNA와 RNA) 등이 독소(toxin)나 담배 같은 오염물질(pollutant)에 노출되는 경우 뿐 아니라 정상 대사 과정에서도 생겨나는 자유 라디칼(free radical)과 반응성 산소 화합물(ROS, reactive oxygen species)에 의한 손상으로부터 보호하는 역할을 한다.
비타민 C는 또한 비타민 E 등의 다른 항산화제를 재생(regenerate)시킬 수 있다.
결핍증 Deficiency
괴혈병 Scurvy
심각한 비타민 C 결핍증은 수 백 년 간 치명적인 질환인 괴혈병으로 알려져 왔다. 1700년대 말, 영국 해군은 오렌지와 레몬 섭취로 괴혈병을 예방할 수 있다는 것을 알게 되었다. 괴혈병은 하루 10mg 밖에 안 되는 비타민 C 섭취로도 예방할 수 있으므로 산업화된 나라에서는 드물지만, 최근의 증례들에 의하면 소아나 식사가 부실한 노인에서 일어난 것이 보고되었다.
영양 권장량 The Recommended Dietary Allowance
비타민 C의 영양 권장량(Recommended Dietary Allowance, RDA)은 2000년 상향 조정 되었는데, 남성, 여성 모두에서 하루 60mg에서 남성 하루 90mg, 여성 하루 75mg으로 증량되었다. 흡연자에서는 비흡연자에 비해 하루 35mg을 더 섭취하도록 하였는데 이는 흡연자에서 담배 연기 속의 독소로 인해 산화 스트레스(oxidative stress)가 더 높고 대개 혈중 비타민 C 농도가 낮기 때문이다.

심혈관 질환 Cardiovascular diseases
700명에서 87,000명에 이르는 다수의 대상자가 3년에서 20년의 기간 동안 참여한 12개의 앞방향 연구 중 7개의 연구에서 다량의 비타민 C 섭취는 심장 질환과 뇌중풍(stroke) 발생 위험이 낮은 것과 의미있는 관련이 있었다.
참가자들의 혈중 비타민 C 농도는 과일및 야채의 섭취와 강하게 관련되어 있었다. 그러므로 많은 연구에서 비타민 C 섭취와 심혈관 질환의 위험의 관련성은 과일과 야채의 다른 성분들의 영향과 분리해서 평가하기 어려우며, 과일과 야채가 풍부한 식사의 중요성이 강조되어야 한다.
암 Cancer
많은 연구에서 신선한 과일과 야채의 섭취가 대부분의 암의 위험을 낮추는 것으로 밝혀졌다. 이러한 연구들은 미국 농업국(U.S. Department of Agriculture)과 국립 암 협회(National Cancer Institute)의 식사 지침에 적어도 하루 5회의 과일과 야채 섭취를 추천하고 있는 근거이다. 많은 뒷방향 연구(후향연구, case-control study)들이 암 예방에 있어서 비타민 C의 역할을 연구하였는데, 이 중 대부분이 다량의 비타민 C섭취가 구강암, 인후암, 성대암, 식도암, 위암, 대장 항문암, 그리고 폐암 발생의 감소와 관련이 있음을 보여주었다. 물론 뒷방향 연구에서 치우침(bias)의 가능성이 더 크므로, 질병에 대한 영양소의 연구에서도 역시 앞방향 연구가 더 무게있게 다루어진다. 일반적으로, 앞방향 연구의 가장 적게 섭취한 군에서도 비타민 C를 하루 86mg 이상 섭취하였고 이들은 암 발생 위험에 차이를 보이지 않은 반면, 적어도 하루 80~110mg의 비타민 C를 섭취한 군에서는 암 발생 위험이 의미 있는 감소를 보였다.1
870명의 남성을 대상으로 한 25년간의 앞방향 연구에서, 하루 83mg 이상의 비타민 C를 섭취한 군은 하루 63mg 이하로 섭취한 군에 비해 폐암이 64% 적게 발생했다.11 대부분의 큰 규모의 앞방향 연구들은 유방암과 비타민 C 복용 사이에 관련을 발견하지 못했으나, 최근 두 개의 연구 결과에서는 비타민 C섭취가 특정한 일부 대상자에서 유방암 위험과 반비례하였다.
백내장 Cataracts
백내장은 미국에서 실명의 주요 원인이며, 나이가 많아짐에 따라 더 흔하고 심각하게 나타난다. 눈의 수정체(lens)에서 비타민 C 농도가 감소하는 것은 사람에서 백내장의 중증도와 관련되어 있다. 일부 연구에서 비타민 C 섭취 증가와 비타민 C 혈중 농도의 증가는 백내장의 위험 감소와 관련되어 있었다. 이러한 연구들에 의하면 비타민 C의 예방적 효과가 나타나려면 하루 300mg 이상을 수 년 간 섭취해야 한다.
납 독성 Lead Toxicity
미국에서는 납 페인트와 납 가솔린의 사용이 금지되었으나, 납 독성은 계속 심각한 건강 문제가 되고 있으며, 특히 도시 지역의 소아에서 중요하다. 임신 중 납에 폭로된 여성으로부터 태어난 영아(infant)들에서 성장과 발달의 장애가 관찰되었으며, 납에 만성적으로 노출된 소아는 학습 장애와 행동 문제를 일으키기 쉽고 지능(IQ)도 낮았다. 성인에서, 납 독성은 콩팥 손상과 고혈압을 일으킬 수 있다. 747명의 노인 남성을 대상으로 한 연구에서, 하루 평균 비타민 C 섭취가 109mg 이하인 대상자들은 그보다 많은 양의 비타민 C를 섭취하는 군에 비해 혈중 납 농도가 의미있게 높았다. 비타민 C 섭취와 혈중 납 농도의 관련성의 기전은 아직 알려지지 않았으나, 비타민 C가 장에서 납의 흡수를 방해하거나 소변으로의 배출을 증가시킨다고 추측되고 있다.
